misaki had a near death experience while her eye surgery, there is a possibility that she is the dead. Even sakakibara can be the dead one as his mother died while his birth, what is he died alond with her and is now the dead one.

i think sakakibara could be the dead one also because he had that lung thing at the beginning, right when he moved into the town. this has got to do something with it.
